From January 2007, Southbroom surgery will be accepting bookings for routine GP appointments via a secure online system. Registration for this service is required and application forms can be obtained below.

Once you have applied and received your registration details you can make appointments using our online booking service. If you have any problems or enquiries about this new service, please contact the surgery.

| Please remember to make a separate appointment for each person to
be seen. Please inform us if you want a longer than usual appointment
for discussion or a medical. Please also note that this e-mail service
is for routine, non-urgent appointments only. Patients will be e-mailed
a booked appointment time and it will be up to the patient to telephone
during office hours to cancel this and re-arrange if the appointment
e-mailed is not acceptable. |

that the time may be offered to another patient. |

| Please make it clear if your problem requires urgent attention.
If you are acutely ill you will always be seen that day, but not necessarily
by your usual doctor. |

| For routine matters please arrange to see your usual doctor, who
will know your medical history. Try and make follow-up appointments
well in advance. |

| Please call at reception or telephone during surgery hours only.
As Monday is the busiest time of the week please avoid telephoning
then for non-urgent appointments, repeat prescriptions or results. |
